This is what a potential patient signs when applying for a spot on the ToothMobile. We don't use a rigorous income schedule to determine need. We understand that there are various circumstances in which a person may find themselves in need of help. Please consider the following before filling out an application:

I understand the dental services offered today are for those who cannot otherwise afford dental care. I attest that the financial hardship of paying for such services would prevent me from receiving them. I further attest I have no discretionary income , no dental insurance, and no other financial means to pay for dental services at this time.

These dental services are a result of a 100% volunteer effort. These services are available as a result of individuals and organizations working together to bring the ToothMobile to San Juan County; this is NOT a Government program. These volunteers take time way from their families and/or their jobs, at absolutely no benefit to them, other than the good feeling they receive from helping others.
So please, if you encounter any difficulty, confusion, or delay in receiving dental services, please be patient and kind to the person helping you, and make them feel good about their efforts to help you.
